Step 1: Call the office to schedule your consult. Next, download your new patient paperwork by clicking here.

Step 2: Email your completed packet and necessary photos to the office using the ecrypted link in your email inbox. (Please be sure to have all documents turned in 24 hours before your appointment).

Step 3: At the time of your scheduled consult, click the "Start a telemedicine call" button to the left to begin your virtual consult with Dr. Shell. (Be sure your using Google Chrome or Firefox as your internet browser).
